Instructor Contract Agreement
Only instructors who have been hired to teach will be required to submit an instructor contract.
Directions: Instructors will click on the seminar options listed above which will lead you to log in with your UT netID and password. Once you submit your part of the electronic contract, an automated email will be sent to who you indicated is your supervisor for them to complete their section. In order to finalize your contract, we must have both instructor and supervisor electronic signatures. If you have any questions or issues accessing the form, please contact Jose Lee-Perez.
Please note, your supervisor will need to create an account in order to access their portion of the contract. More directions will be provided in the automated email they receive.
